Synopsis
PlanCare is a healthcare-at-home platform that enables older adults to self-manage their care and support, giving them more choice and control and access to more services and providers at a lower cost than the traditional care model. PlanCare connects to the over 65’s Healthcare-At-Home ecosystem, which includes those requiring support, their family members and caregivers, as well as their funding source. The platform brings everyone together to reduce the stress of coordinating supports and provides clarity on supports, including fund and budget management, whilst monitoring the well-being of their users.
